To ensure timely consultation and appropriate triage of the referred patients, please follow the following guidelines:

• For routine outpatient nephrology consultations (>2 weeks), please complete the one-page referral form below and we will schedule the patient with one of our providers as soon as possible.
• For urgent nephrology consultations (1-2 weeks), please complete the referral form and call our office so that our front office staff can prioritize scheduling the appointment without delay.
• For emergent nephrology consultations (<1 week), please complete the referral form and call our office. One of our nephrology providers will review the chart and overbook the patient within the desired time interval.

Our nephrologists are always available to speak directly with any of your providers if urgent nephrology questions or issues arise. During business hours your staff can contact our office, and we will connect the provider with one of our nephrologists. After hours and on weekends, the provider can call Mercy Medical Center in Roseburg (541-673-0611) and ask to speak with the nephrologist on call for Roseburg Kidney Care. Our physicians provide 24/7 comprehensive inpatient call coverage at the hospital.
